ENHANCE: Development and evaluation of a targeted, integrated, coherent and people-centred approach to the management of multimorbidity in South African primary healthcare

Description:

Overall aim: To co-produce and evaluate a scalable health system intervention and implementation strategy for improved management of commonest configurations of multiple long-term conditions (MLTCs) in SA that balances clinical evidence, local policies, patient and community preferences and health system constraints. The study is in collaboration with Kings College London and the Knowledge Translation unit at the University of Cape Town. It comprises six work packages, with UKZN leading WP4, which entails formative work designed to inform the implementation strategy for the intervention. The intervention comprises strengthened health promotion materials in the waiting rooms, an integrated set of clinical guidelines for use by Professional nurses/doctors for the commonest configurations of MLTC, and outreach to households by community health worker teams to provide support using a patient held diary. The intervention is being tested through a cluster randomized control trial in KwaZulu-Natal and the Western Cape province, with the UKZN team leading the trial in KwaZulu-Natal.

Current Highlights/Updates:

The trial is currently underway with baseline data collection completed, and 6-month follow-ups in progress.
